Cancellation Policy
I know life happens. I do, however, have a 24-hour cancellation policy as the time is reserved specifically for you.

- A cancellation within 24 hours of your scheduled appointment time, will likely incur half of your session fee.
Please call / text me at 408.905.7066 to discuss. - A no show or no call is made to let me know you are not planning on keeping your appointment.
- You will be responsible for the full amount of your session. This fee must be paid before your next
scheduled appointment.

- If you cancel multiple appointments within 24-hours, I will require prepayment for your next appointment(s).

I currently do not require prepayment.
Payment Policy
I require payment after your session. I accept cash (exact amount please), check, major credit cards (Visa, MasterCard,
Discover, American Express), Apple Pay, Google Pay, PayPal, Samsung Pay, Venmo, and Zelle.
Client Intake Form
A complete Client Intake Form will be required to be seen.
- The Client Intake Form can be completed online. The form is sent to your email address when you schedule your
appointment and again one day before your appointment if you have not completed it. Please make sure to
double-check your email address. - Client Intake Form can be downloaded and printed. Feel free to complete it at your convenience and bring it
with you to your appointment. - You can complete it at the office. Once you have parked your car please call/text me to say that you have
arrived. - I will walk to your car and ask that you complete the form in your car, or you can elect to complete it in
our reception area. Please be aware, the time you take to complete the form will be taken from your massage
session.
COVID-19 Precautions
- I will use a clean face mask and an apron to cover my work clothes during your session. I will change both for
each client before their appointment. - I will continue to use proper hand hygiene.
- I will disinfect the treatment room before and after seeing each client.
- All used linens will be stored in a closed bin in another room.
- I will use the HEPA air filtration system between clients in the treatment room.
- I ask that you text or call me when you park and wait in your car. I will call or text you when I can have you
come up to our reception area. - Due to the physical distancing protocols put in place, we ask that your family and friends stay outside or wait in
the car for you during your session. - You will be required to wear a face mask or face covering to enter our office. If you cannot wear a face mask or
face covering, please let me know PRIOR to your appointment. Please either forward a doctor’s note with this
restriction stated on it with start and end dates listed to anna@opentoserenity.com or bring in a copy with you
to your appointment. - I may take your temperature using a non-contact thermometer when you reach the reception area. If your
temperature is above 100.4◦F [38◦C], we will reschedule your appointment with no penalty charged. - I will ask you to use hand sanitizer. It will be provided for you when you walk into our office. There will be hand
sanitizers throughout the office and in the treatment room for your use. If you would rather wash your hands, I
will escort you over to our wash basin. - You and I will both use face masks throughout the session regardless of COVID-19 vaccination status.
- I can provide you with a cotton face mask or disposable face mask to use during your session if you don’t have
one. - After your session, I will ask you to use the hand sanitizer or provide you with the opportunity to wash your
hands upon leaving the treatment room. We will then meet in the reception area for post session review,
scheduling your next session, and payment. - Payment options will include non-contact methods. If you plan to pay by cash, please have the exact amount. You can visit my website for pricing or ask me. I have special pricing for clients over 65 years of age,
Military/Veterans, and Healthcare Workers that are NOT listed on the website. - I will use the HEPA air filtration system between appointments and during the disinfecting of the rooms.
- Sessions will now be staggered with an increased amount of time between appointments to accomplish the
cleaning/disinfecting protocols. - All clients are seen by appointment only.
